Directions & Checklist for BEA Scholarship Applicants

Scholarships for Academic Year 2011-2012

 

This checklist is for your use only.  It is intended to assist you in making certain that your application has been properly prepared and submitted.  Do NOT return this checklist with your submission.

 

__________ 1.  Four completed and collated copies of the main application form (Form #1).  Staple each copy securely.  You can fill out and print off the electronic application form on the BEA website. 

(THAT MEANS ONE TYPED ORIGINAL AND THREE PHOTOCOPIES.)  The application MUST be typed.  Keep an additional copy for your files.  APPLICATIONS NOT TYPED WILL BE DISQUALIFIED.  Remember to sign question #1 regarding full-time status for the entire academic year.

 

__________ 2.  Waiver sheet (Form #2)

Make four copies of this form and attach them as the last page of the main application sets.  If you choose not to sign the waiver sheet, please so indicate at the bottom of the sheet, sign your name to your refusal statement, and return it.  In either case, the Waiver must be included with your application.

 

__________ 3.  Letters of reference

Three letters of recommendation should be mailed directly to Dr. Orlik by their authors.  Check with your references to ensure that these letters reach Dr. Orlik by the October 12, 2010, deadline.  At least one letter must be written by a college electronic media faculty member.

 

__________ 4.  College transcripts

If you have attended more than one school, a transcript is needed from each institution.  (If work at a previous school is included in your present transcript, that will suffice IF all courses taken, the individual credit hours, and specific grades are given.)  Unofficial student copies of transcripts are acceptable if clearly legible and if your name appears on them. (Some on-line transcripts lack student name and are unacceptable.)

Please do NOT send tapes, resumes, photos, etc.  These will not be considered by the committee.

 

SUMMARY:   Send FOUR typed and stapled copies of Application Forms 1 and 2 to be received by Dr. Orlik by October 12, 2010, by close of business.  Please use 8½” x 11” white paper.  Make certain that all college transcripts are also received by October 12, 2010

by close of business.  Double-check your three references to ensure they have sent their letters directly to Dr. Orlik to be received in his office BY CLOSE OF BUSINESS on October 12 2010. BEA IS NOT ABLE TO PURSUE MISSING ITEMS.   This is your responsibility.  Incomplete applications will be disqualified.
